Athletic Clearance

SUBMIT ALL FORMS ONLINE THROUGH ATHLETICCLEARANCE.COM

In order for students to begin participating in athletics, they must have completed the athletic clearance process which can be found online at Athleticclearance.com.

Athletic Clearance is required for ALL students wishing to participate in sports:

1. Athletic Clearance: Create Online Account
2. NFHS Courses: COMPLETE CERTIFICATES REQUIRED.

• Concussion in Sports
• Sudden Cardiac Arrest
• Heat Illness Prevention

*Sports Physical: EL 2 physical form must be used and signed by the Doctor as well as by the parent and student. Physicals are valid for 365 days. Upload page 4 of the EL2 (physical form) with all signatures.
